Our history is a unique story of a community coming together to improve the lives, return hope and prevent diseases. For almost two decades friends and neighbors, including you, have provided innovative healthcare to the uninsured and underserved residents through supporting the Community Medical Clinic (CMC) of Kershaw County.
In the past year we emerged as:
- The only free clinic in the nation to measure out comes with the Hub and Pathways Model, a robust database backed by the IHI Quality Improvement Advisory Program
- One of three free clinics operating a School-Based Health Center in the United States.
- The only free clinic to receive a Duke Endowment Access Heath Grant
- The first free clinic in South Carolina to use a practitioner preceptor model.
Our volunteers and donors are leading the way to better health in not only Kershaw County, but the state of South Carolina. At CMC, we know that in individual’s health is not just about a list of symptoms; it’s about their environment, where they work, eat, sleep and live. Together we are reducing the missed workdays, hospital readmissions, and medication management mistakes.
